The Effect of the Impingement on Polyethylene Wear after Total Hip Arthroplasty

Abstract
Purpose: To assess the effect of the prosthetic impingement on polyethylene wear and to clarify the cuasative factors of impingement.

Materials and Methods: Comparative study matched by sex, age, size of cup and head diameter was performed between a group of twenty-two liners with evidence of impingement and a group of twenty-two liners without evidence of impingement. A clinical and radilogcial assessment was performed. Data analysis was performed with use of Student t-test.

Results: The preoperative Harris Hip score(HHS) of the impingement group was significantly less than that of the non-impingement(67.1 versus 85.7, p=0.017). There was no significant difference between two groups at the most recent follow up HHS. Additionally, the degree of abduction in the impingement group was significantly smaller than in the non-impingement group(29.3 versus40.4, p=0.001). The mean rate of linear wear I the impingement and non-impingement groups was 0.743 §®/year and 0.430 §®/year(p=0.042), respectively; the volumetric wear was 1954.85 §§/year and 1006.13 §§/year(p=0.049), respectively; and the head-neck ratio was 1.808 and 1.958(p=0.048), respectively. These results were statistically significant.

Conclusion: Prosthetic impingement following THA may result in a duformation of the poyethlene liner and an acceleration of polyethylen liner wear and may in turn significantly influence the longevity of THA. For the prevention of impingement between the components, it is imperitive to select an implant the largest possible head-neck diameter ratio.
